FOLLOW-UP RECOMMENDATION: Obtain additional lung screening CT images and/or comparison to prior chest CT examinations is required to provide a complete interpretation of findings.

EFUN - IMMEDIATE DIAGNOSTIC WORKUP

PROCEDURE: Low dose lung screening CT performed as either a baseline screen or part of periodic screening - OR - Low dose chest CT performed as part of a recommendation to obtain a follow-up CT from a baseline screen or part of periodic screening LDCT between 4 - 9 months prior.

IMAGING ASSESSMENT: Suspicious / Very Suspicious (Lung-RADS® Score 4A, 4B, or 4X)

FOLLOW-UP RECOMMENDATION: Obtain additional LDCT in 3-months; PET/CT - OR - Chest CT; PET; Tissue Sampling.

EFUN - INTERVAL FOLLOW-UP

IMAGING ASSESSMENT: Probably Benign (Lung-RADS® Score 3)

FOLLOW-UP RECOMMENDATION: Obtain interval LDCT within the next 3-6 months - OR - return to annual screening with LDCT at some date less than 12 months from this most recent interval LDCT procedure date. (Recommended F/U date provided in the communication.)

EFUN CL - INTERVAL FOLLOW-UP

PROCEDURE: Low dose lung screening CT performed as either a baseline screen or part of periodic screening - OR - Low dose chest CT performed as part of a recommendation to obtain a follow-up CT from a baseline screen or part of periodic screening LDCT between 4 - 9 months prior.

IMAGING ASSESSMENT: Probably Benign with Other Modifier (Lung-RADS® Score 3 S-Modifier)

FOLLOW-UP RECOMMENDATION: Obtain interval LDCT within the next 3-6 months - OR - return to annual screening with LDCT at some date less than 12 months from this most recent interval LDCT procedure date. (Recommended F/U date provided in the communication.) Contact healthcare provided to obtain further follow-up guidelines/recommendations based on clinical findings.

NO LETTER

PROCEDURE: Low dose lung screening CT performed as either a baseline screen or part of periodic screening - OR - Low dose chest CT performed as part of a recommendation to obtain a follow-up CT from a baseline screen or part of periodic screening LDCT between 4 - 9 months prior.

IMAGING ASSESSMENT: Negative, Benign Appearance or Behavior (Lung-RADS® Score 1 or 2), Probably Benign (Lung-RADS® Score 3), Suspicious / Very Suspicious (Lung-RADS® Score 4A, 4B, or 4X), Incomplete (Lung-RADS® Score 0), Other (S-Modifier added to Lung-RADS® 0-4)

FOLLOW-UP RECOMMENDATION: Instructions to provide NO communication to examinee based any imaging scenario per client-specific instructions - OR - communication determined to conflict with providing appropriate to examinee based on the most current information available in the continuum of their care (i.e. additional procedures/follow-up already obtained, time from procedure greater than 6-months, etc.).

Typical Letter Scenarios - Reminder Type

Communication with examinees is typically provided in accordance with the imaging scenario associated with each specific letter type.

RFUR - ROUTINE FOLLOW-UP EXAM REMINDER

MOST RECENT PROCEDURE: Low dose lung screening CT performed as either a baseline screen or part of periodic screening - OR - Low dose chest CT performed at some date less than 12 months previously following an initial low dose lung screening CT, with a recommendation to return within the time frame matching the current follow-up criteria used for generation of the specific reminder letter batch.

IMAGING ASSESSMENT: Negative, Benign Appearance or Behavior (Lung-RADS® Score 1 or 2) - OR - Probably Benign (Lung-RADS® Score 3) assessed for the intended purpose of returning examinee back to a specific low does screening interval protocol.

FOLLOW-UP RECOMMENDATION: Continue annual screening with LDCT in 12 months from prior current screening CT procedure date. (Recommended F/U date provided in the communication.) - OR - Return to annual screening with LDCT within the recommended follow-up period specified at most prior low dose chest CT. (Recommended F/U date provided in the communication.)

EFUR - EARLY FOLLOW-UP EXAM REMINDER

MOST RECENT PROCEDURE: Low dose lung screening CT performed as either a baseline screen or part of periodic screening.

IMAGING ASSESSMENT: Probably Benign (Lung-RADS® Score 3)
